"The long-term conditions (LTC) part of the block is about the lived experience of the person with a long-term condition and their journey through the age continuum and multiple referral pathways. The multi-professional faculty will focus on the identification, assessment and management of patients with long-term conditions and disabilities and by exploring the holistic role of the medical practitioner in both coordinating and contributing to the on-going care of patients in an integrated manner."
